Incident Reporting and Worker's Compensation
Did an incident occur during service? Complete an Incident Report Form for ANY physical or vehicle accidents, close calls, etc. If you are pursuing medical care and want to apply for Worker's Compensation, complete an "Incident Report Form" AND have the physician complete a "Letter from the Treating Provider".
