Incident Reporting and Worker's Compensation
It is recommended to keep physical copies of these forms in the event of any incidents that occur. Complete an Incident Report Form for ANY physical or vehicle accidents, close calls, etc. If the member is pursuing medical care and wants to apply for Worker's Compensation, complete an "Incident Report Form" AND have the physician complete a "Letter from the Treating Provider". Mid-Term Evaluations All members who serve for 6 months or more are required to complete a mid-term evaluation. ACC Staff will reach out to you 2 weeks prior to the due date with information. The member will complete their side of the form, then send to the supervisor to complete. The goal is to schedule a one-on-one sit down meeting to discuss member strengths and any areas that could use support. The form must be signed and returned to ACC staff by due date. |
